The application / enrollment form for Aadhaar UID number may differ in language and other  aspects for your area. The official form from UIDAI have above fields, but in some states there are additional pages along with the above form for providing additional information.

The other documents required along with the form are:

  • Proof of Identity (PoI)
  • Proof of Address (PoA)
  • Date of Birth (DoB)

Photocopies of the form (both black n white and colored) are accepted at some camps and at others they insist for forms issued by them. As per our knowledge the UIDAI haven’t clarified weather Photocopied and downloaded forms are acceptable. A form acceptable throughout India would be a better option. As per UID norms, the application can be submitted anywhere in India.


The Government of Andhra Pradesh has started issue of Aadhaar Smart Cards for the people of the state. The issue of cards for Hyderabad and Ranga Reddy districts have been started long back and the process is still undergoing. The Government has also proposed to start issue of Aadhaar Cards in five more districts of the state initially and this has been announced in the Budget Speech of 2011-12. To proceed with online registration, you need to enter your Ration Card No and get the details before entering family members particulars. Documents and Certificates Required for Aadhaar Enrollment

This article will provide you information on the documents or certificates to be attached or produced in the process of enrollment for Aadhaar Cards. Though there are small variations in Aadhaar requirements at the state level in different states, following are the general certificates to be produced by all the members of the family for Aadhaar Unique ID enrollment. There are only three important proofs or copies of certificates you need to provide while applying for Aadhaar cards. These are Proof of Address, Identity and Proof of Age (This may be optional). Following are the accepted proofs:


For Proof of Name and Identity:

Aadhaar cards will be considered for issue and implementation of various schemes like pensions, Arogya Sri, MGNREGS, LPG Connections, Student fee reimbursements and scholarships. Govt. Housing and subsidized Rice schemes under PDS. It is estimated that Govt. of AP has already enrolled six lakhs people under Aadhaar scheme.

First phase of Aadhaar enrollment has been under progress in Hyderabad, Rangareddy, East Godavari, Srikakulam, Adilabad, Anantapur and Chittoor. It is estimated that about 50 percent of enrollments were completed in these districts. But the delivery of unique number cards has been delayed due to logistic problems. The second phase of enrollment process for Aadhaar will commence from August 2011 in the remaining 16 districts of the state.
